Support Coordination: Your Plan, Your Way

If you are looking for high quality support for your NDIS plan, talk to the Support Coordination team at Windgap.

Our focus is to get to know you, your needs and goals, and ensure you get the very best out of your NDIS plan.

We know that your needs may change from time to time, so we advocate for you and any changes you need for your plan, and help solve any problems.

We help you build your ability to make choices so you can lead your life the way you want, and coordinate your support and social network in the community.

Support Coordination at Windgap:

Support Coordination focuses on supporting people with a range of disabilities such as physical, intellectual and psychosocial disabilities.

  • We help you build your capacity to exercise choice and control, to coordinate your support network, and access your local community.
  • We go above and beyond to make your life simpler and easier
  • No problem is too big. We support you with every step of your NDIS journey
  • We ensure your journey with the NDIS is as smooth as possible with regular follow-ups and ongoing support.

As an NDIS participant you are never locked in, even if you are currently receiving Support Coordination from another provider you can switch NDIS providers or Support Coordinators at any time.

All Support Coordinators at Windgap are trained professionals with an Allied Health background and have extensive knowledge of local disability services, so you (or the person you care for), will receive qualified support, advice and a range of choices to suit your needs.

Specialist support coordination funding is given to people with complex needs so that support can be provided by a qualified and experienced practitioner. This might include professionals such as an occupational therapist, psychologist or social worker to name a few.

Windgap has been great in supporting me as I navigate the NDIS. Communication is always answered. Always a positive experience.

It’s been a godsend having my Support Coordinator to navigate the minefield that is the NDIS. She is a caring & helpful person and nothing is too much trouble for her.

The Support Coordinators at Windgap are exceptional, caring and very responsive. Really appreciate the extra effort as I could not have managed the complexity during these tough times.

FAQs – What is an NDIS plan?

The National Disability Insurance Scheme (NDIS) is Australia’s support system for people with a disability. Each individual will receive a unique funding package that will assist them to meet their NDIS goals. To find more information about applying for an NDIS plan refer to the NDIS website.

What is Specialist Support Coordination?

Specialist Support Coordination may be funded when the NDIS has assessed that a participant has complex and or high support needs. Your specialist support coordination will be provided by a qualified and experienced practitioner such as an occupational therapist, psychologist or social worker.

Windgap has a team of qualified specialist support coordinators who will be able to manage challenges in your support environment in areas such as health, education and justice services.

Is Support Coordination free?

Unfortunately not. Our support coordination team can only assist people who have support coordination funding included as part of their NDIS package. This funding is included as a separate category under capacity building.

Support coordination funding is funded at an hourly rate and is allocated according to you or your loved one’s individual requirements. If you have received this funding, your support coordinator will sit down with you to work how to utilise the funding in the way that best suits you.

Didn’t receive funding for Support Coordination?

If you didn’t receive support coordination in your plan and you believe that you need it, our team can provide a letter of support for your next NDIS plan to your local area coordinator to help your voice be to heard.
